SERVICES TERMS & CONDITIONS

The terms and conditions set forth below apply to all services provided by Charee Smith d.b.a. Charee Janelle (“Provider”) to any individual or group (“Client”) and serve as the agreement for the service(s) provided by Provider to Client. In return for the fees payable by the Client or by a third party on their behalf, the Provider agrees to furnish the service as described below and in accordance with the terms and conditions set out below.

1. Format of Sessions or Classes: Services furnished are offered at Provider’s home sanctuary/studio location in Morgantown, Pennsylvania, or in some cases, also by Zoom video conferencing, and at the client’s home depending on geographic location.

2. Dates and Times of Sessions: The date and time of an initial session and any subsequent sessions shall be selected by Client through Provider’s appointment scheduling program. The Client is responsible for arriving to appointments on the agreed upon dates and times. At least 48 hours’ notice should be given if a client needs to rearrange an appointment. All provider services that have been paid for in advance must be used within 6 months of the payment or the fee is forfeited.

3. Payment Terms: Fees are payable in advance of each service through the Provider’s appointment scheduling program’s secure payment processor. Receipts and/or invoices are available upon request and will be sent by e-mail.

4. Refunds: Refunds shall be given to the Client for unused services/sessions/classes only if the Client has furnished at least 48 hours’ notice of cancellation to Provider.

5. Confidentiality: Personal information supplied by a Client for services will be treated as confidential. It will not be disclosed to a third party without the Client’s prior permission except when and where required by law or where action might be necessary to prevent harm to Client or another party.

6. Clients’ Responsibilities: Provider accepts no responsibility for client outcomes or liability for any loss incurred as a result of using Provider’s services. The Client may be assigned tasks or exercises to complete between appointments. While the Client is under no obligation to complete them, not doing so may slow the Client’s progress in gaining the desired outcomes.

7. Disclaimer: The client understands and agrees that the Provider’s services are not intended to be a replacement for any other type of professional medical care that the client may deem necessary for her/his medical well-being.
